Whenever we begin working with a new client, we are required to process Intake Paperwork which provides us with your information in addition to consent for counseling. You can click on the Intake Forms below to print out and complete prior to your session. You also have the option of arriving 20 minutes prior to the start of your scheduled session to complete the forms provided to you in our office.

If the client is a child or adolescent under the age of 18, either or both parents may be required to be present and sign the consent forms prior to the intake appointment.
